This requirement is separate to that of the Letter of Recommendation and must be issued by your employer’s HR office and show your start and finish dates (or confirm that you are still employed).
Note: If you were directly employed by the U.S. government, a copy of the employment contract may be submitted for consideration.
We have received a letter of recommendation for you signed by OOOOOO. However, he did not indicate in his letter of recommendation if he is your direct supervisor. Under section 1244 of the Defense Authorization Act, applicants are required to have a letter of recommendation from their direct supervisor. Please have him respond to this email and indicate that he is your direct supervisor. You must answer each question completely and NOT skip any questions. Questions that are Not Applicable should be so marked. Questions 9, 10, 12 and 13 should NOT be marked as Not Applicable. You should either provide an answer to those questions or mark them as None. Please note that questions 12, 15 and 17 need to have beginning and ending dates.

The applicant must submit a scanned copy of their valid Iraqi G or A series passport. If no passport is available please provide a detailed explanation explaining why the applicant does not have a passport.
The applicant must submit a brief statement describing the threats they have faced as a result of their employment on behalf of the United States government in Iraq.
