To file your SIV petition you must send the following items to USCIS:
1. A completed Form I-360 -

2. A copy of the passport, birth certificate or national identification card showing that the applicant is a national of Iraq, along with a certified English translation, if the document is in a foreign language.
3. A copy of the positive recommendation, confirming employment of not less than one year beginning on or after March 20, 2003, normally from the U.S. citizen who has directly supervised the Iraqi employee or supervises the company for which the worker is employed.
4. A copy of the attached “Chief of Mission Approval for SIV Status” letter.
