Please find attached the SIV guidelines and SIV FAQ's, which we hope
will provide clear guidance about the documentation we require for this
part of the SIV program, as well as an overall summary of the entire SIV
process.
There are three important things we ask you to note when preparing an
application for Chief of Mission (COM) Approval to apply for the SIV
program:
1. When completing the DS-157 form, please be sure to include the date
ranges (beginning and ending) for all employment, academic and military
history listed. This can be handwritten as long as the writing is clear
and legible. If the applicant has any foreign military service, they
must provide as much detail as possible about their service and if they
served for more than eighteen months, they must also provide a scanned
copy of their military service record book, and an English translation;
2. SIV applicants can only include their spouses and unmarried children
under the age of 21 on their applications; and
3. The applicant must have been employed by or on behalf of the US
Government in Iraq for no less than one full year, without exception.
