Fees: ($170) by check per each person 15 years of age or older and ($80) by check per each person under 15 years old. Payment should be made by certified check payable to IOM (International Organization of Migration) certified from the National Bank of Iraq. The National Bank of Iraq is located on Al-Sadoon St. – Ferdows Sq. (near the gas station / opposite to Al-Alawiah Club (Sheraton and Le Meridian Hotels).
Applicants must provide the following documentation when they appear for their examination:
• Recent passport sized photographs (4 copies)
• Color photocopies of passport (3 copies)
• Color photocopies of identification card (3 copies)
• Appointment letter showing that you have a scheduled visa interview (3 copies)
• Immunization records if available
• All past medical reports and current medications, if any

Fees: ($170) U.S. Dollars Cash per each person 15 years of age or older and ($85) U.S. Dollars Cash per each person less than 15 years old. U.S. Cash will be the only accepted form of payment.
When appearing for examination at Wamar Clinic, applicants must provide the following documentation:

• Color photocopies of passport (3 copies)
• Color photocopies of identification card (3 copies)
• Appointment letter for scheduled visa interview at the Consular Section of the U.S. Embassy (3 copies)
