قبل يومين استلمت كوم بروفايل وكما هو معروف يجب ارسال المستمسكات الى نبراسكا ومن ضمنها i 360 وبدأت اقرء بموضوع الاخ جاك حول طريقة ملئ الاستمارة وفوجئت بتضارب المعلومات حول حقل الخاص بفقرة co adress الموجدة في بعض البارتات البعض يقول انها تخص المكتب الذي سوف ترسل منه الاوراق والبعض يقول اسم الشركة وبصراحة لم اصل الى نتيجة
ولو سلمنا ان هذه الحقل يخص اسم المكتب الذي سوف ترسل منه الاوراق ناتي الى رسالة المرسلة لي مع كوم بروفايل الي تخبرني انه يمكن لي ان ارسل الاوراق عن طريق البريد الالكتروني ومنطيني البريد الالكتروني زين اذا دزيت الاوراق عن طريق البريد الالكتروني هذا الحقل شكتب بي لانه انا ما راح ارسل من مكتب لدي اج ال انما راح اريل عن طريق البريد الالكتروني
اخواني بصراحة تحيرت بالاستمارة ومال اعرف شنو الي اتركة وشنو الي املي لهذا اخواني احتاج مساعتدتكم وخصوصا اصحاب الخبرة بهذا البرنامج
احواني انا اعزب وليس لدي كفيل لهذا اكيد اكو فقرات ما تحضني لذا الي ارجو منكم مساعدتي بكيفية ملئ الاستمارة

Thank you for your interest in the Special Immigrant Visa Chief of Mission Approval (SIVCOM) Program.
Congratulations, your Chief of Mission (COM) application was approved for the Iraqi Special Immigrant Visa (SIV) program. Attached to this e-mail is an electronic copy of your COM approval letter.
You must now submit the following package of documents directly to the U.S. Citizenship and Immigration Services’ (USCIS) Nebraska Service Center:
1.) A completed I-360, Petition for Amerasian, Widow(er), or Special Immigrant. The I-360 and the I-360 instructions can be downloaded from the following website:
http://www.uscis.gov/i-360 . NOTE: To be properly filed, the Form I-360 must include your original signature.
2.) A copy of your passport, nationality certificate (Shahadat Jensiyah), or national identification card showing that you are a national of Iraq, along with a certified English translation, if the document is not in English.
3.) A copy of the recommendation from your supervisor that you sent to the NVC when you applied for COM approval.
4.) A copy of your COM approval letter, which should note that the COM or his or her designee:
* Determined that you have been employed by, or on behalf of, the U.S. Government on or after March 20, 2003, for a period of not less than one year;
* Conducted an independent review of records maintained by the U.S. Government or hiring organization or entity to confirm employment and determined that you have provided faithful and valuable service to the U.S. Government; and
* Determined that you have experienced or are experiencing an ongoing serious threat as a consequence of your employment by the U.S. Government.
5.) If you are in the United States when you file the petition, a copy of the front and back of your Form I-94, Arrival-Departure Record.
To submit your SIV package to USCIS’ Nebraska Service Center you may scan and e-mail your petition with the required documents (preferably in pdf format) to the following e-mail address: NSCI360SIVAPP@uscis.dhs.gov
NOTE: If you submit your I-360 by e-mail, you must bring the same original, signed Form I-360 that was submitted to the Nebraska Service Center with you to the consular interview. Otherwise, you may send the original I-360 and required documents by either regular mail or overnight delivery, to the appropriate address below.
