We reviewed the forms and documents submitted with your Chief of Mission (COM) application. The following documents, however, are still required to complete your application:

Please review the attached Applicant Guidelines for Chief of Mission Approval for specific instructions on how to obtain and complete the above requirements.
In addition to the above required documents please submit the following information:
While reviewing the form DS-157, Supplemental Nonimmigrant Visa Application, we noted that the following questions are incomplete: 10.
Please read and follow the attached instructions carefully when completing the form DS-157. If the form does not provide sufficient space for a response, additional sheets of paper may be used.
The form DS-157 can be downloaded from the following website:

IraqSIVApplication@state.gov .
While reviewing the Human Resources (HR) letter submitted in support of you Chief of Mission (COM) application, we noted that the verification of employment letter was not issued by RTI’s HR department.
If RTI does not have an HR department, you may submit a letter on company letterhead signed by the Project Manager, Vice President or any senior official.
In order for the Chief of Mission (COM) designee to make a determination on your COM application, the following information is required:
* Please indicate what you were doing from 1999 to 2002 and from 2011 to Present.
* Please include all education, employment or travel during the time period indicated above.
NOTE: If you were unemployed or not attending school, please send a brief explanation.
Please e-mail a scanned copy of the above forms and documents to

1 ايطلبون الفترة من ال1999 ال2002 وين قضيتها : السوال وين اكتب هذة الشي بالفورمة نفسهة لو اكتب الهم رسالة اجاوبهم مع العلم اني ذكرت هذه الفترة في حقل العسكرية (من فبراير1999 ال فبراير 2001 )
2 الفترة من تاريخ 2011 اللي انتهة بية العقد الخاص بالمنظمة الى هذا الوقت ؟؟؟؟ وين اكتبة
