Applicants must present the following at the time of their medical appointment:
· Fees: Payable in U.S. dollars, cash only – USD $160 per person 15 years of age or older, and USD $75 per person under 15 years of age.
· Recent passport photographs (4 copies)
· Color photocopies of passport and jensiyah (3 copies)
· Appointment letter from NVC or email from the Embassy confirming your appointment
· All past medical reports and current medications, if any
· Vaccines history reports and documents, if available
