I've got my COM one year ago, but the questions are the same.
here is what my supervisor got:
Dear Mr. XXXXX,
We are currently processing XXXXXXXXXXX’s application request for Chief of Mission (COM) Approval for the Special Immigrant Visa (SIV) Program at the U.S. Embassy in Baghdad.
When SIV applications are received, we are required to verify the employment claims of the applicant. The requirements state that the applicant must have been employed by or on behalf of the U.S. government in Iraq, for not less than one year after March 20, 2003, and have experienced or are experiencing an ongoing serious threat as a consequence of that employment.
Please confirm, via reply to this email, the following:
ü The attached letter of recommendation is genuine;
ü The individual named in the letter of recommendation, and in the subject line above, remains or resigned in good standing with your organization;
ü The individual named in the letter of recommendation, and in the subject line above, provided faithful and valuable service to the United States government during their employment;
ü You, as the recommending supervisor, are an American citizen; and
ü The SIV applicant, to the best of your knowledge, presents no threat to the national security or safety of the United States.
