Thank you for your interest in the Special Immigrant Visa Chief of Mission Approval (SIVCOM) Program.
We reviewed the forms and documents submitted with your Chief of Mission (COM) application. The following documents are still required to complete your application:
Please submit a brief written, signed and dated statement describing the threats you faced as a result of employment on behalf of the United States government in Iraq. Please be advised that your previously submitted threat statement was not dated.
While reviewing the form DS-157, Supplemental Nonimmigrant Visa Application, we noted that the following questions are incomplete: 10 and 12.
Please read and follow the attached instructions carefully when completing the form DS-157. If the form does not provide enough space for a response additional sheets of paper may be used.
The form DS-157 can be downloaded from the following website:

Please provide a letter of recommendation from your recommending supervisor. The letter must be from a direct U.S. citizen who knew you personally. It must cover the same period of employment covered by the HR employment verification. The letter should be dated and signed by your supervisor. It must contain:
* Name of applicant
* Date of birth
* Badge number
* Job title
* Job location
* Start date of supervision
* End date of supervision
* Supervisor's name, title, corporate or U.S. Government/Military e-mail address and phone number;
* Justification for recommending you for COM approval, i.e. that you provided faithful and valuable service to the United States Government,
* The supervisor's opinion on whether you pose a threat to the national security and safety of the United States; and
* The recommender is or was your direct supervisor.
If the above recommendation cannot be obtained from a U.S. Citizen supervisor please carefully review the website below for guidance.

1- ورقة تكتب بيهه انت متهدد نتيجة عملك السابق وية القوات الامريكية ولازم بالنهاية تكتب التاريخ وتوقعهه بايدك وترجع تسحبهه سكنر وتدزهه.
2- عندك استمارة ds157 ارجع راجعهه لانه عندك فقرتين 10 و 12 ممجاوب عليهه.
3- تدزلهم رسالة تزكية من مسؤولك المباشر ولازم يكون امريكي الجنسية ولازم يكتب الفتره الي اشتغلت بيهه ويا ولازم تكون نفس الفتره الموجودة باستمارة ال HR ولازم يكتب انه كان مسؤولك المباشر ويكتب انت متشكل تهديد او خطر على الامن الوطني للبلاد ولازم يكون بيهه تاريخ وتوقيع لا ينساهه.
ونصيحتي الك اخوية ترسل هذه المعلومات بايميل لمسؤولك الي يكتبلك الريكومنديشن حتى على اساسهه يكتبك التزكية اخذ هذه المعلومات لمسؤولك بايميل.

كلام الاخ dani مظبوط واني نفس الحاله ترسل المعلومات الى السوبر فايزر مالتك طبعا المعلومات عنه هو شخصيا يكتبه ويوقع عليه واهم شي يرسله عن طريق ايميل رسمي مو ياهو او هوتميل او جيميل بعد ان ترسللهم رسائل التهديد اذا عندك واذا ما عندك اذكر الهم اي شي ممكن يساعد قضيتك واذا السوبر فايزر يذكر برسالته انك مهدد وحياتك في خطر يكون افضل ولا ينسه يذكر انك لاتشكل خطر على الامن القومي المهم بعد ان تكمل هذي المراحل اني بالنسبه الي دزولي رساله كالوا انتظر لمدة ثلاثة اشهر قضيتك تحت المعالجه حاليا بعدلي شهر والله كريم ادعولك بالتوفيق انشاء الله اخي العزيز نسيت اكلك شغله مدة العمل السوبر فايزر عندما يذكره لازم متقل عن سنه
