REQUEST FOR EVIDENCE:
1. The submitted Form I-360 petition did not contain your signature. Therefore, please now submit the I-360 signature page which includes your original signature. Please note that we cannot accept a photocopy or PDF of the signature.
You do not have to sign in front of a USCIS or Consular Official. Please submit just the signature page with this cover sheet. (If the entire petition is submitted again, the mail contractor may misinterpret this as a second filing and reject the information sent.)
2. You have listed a foreign mailing address on your petition. If available, please provide a U.S. address or an APO AE address, as well as the name of the individual who will receive mail for you at that address (such as the individual who helped you gather documents for the SIV program). It would also be helpful if this individual could provide his/her email address. The address may be used when USCIS sends you official/secure correspondence from USCIS.
Your response to the requested information must be received in this office by November 3, 2011. If you do not respond to this request within the time allowed, a decision will be made based upon the evidence of record.
