MEDICAL EXAMINATION INSTRUCTIONS
Every immigrant visa applicant is required to undergo a medical examination by a physician authorized by the U.S. Embassy in Baghdad, Iraq. Only two clinics are authorized to conduct medical examinations – please see the information below. Please contact the clinics directly to arrange an appointment. Applicants must pay for all expenses associated with the medical examination. You may also require immunizations prior to the visa issuance.
International Organization for Migration (IOM) Baghdad
Appointments are available Sunday to Thursday. The clinic’s working hours are 9:00 am to 5:00 pm.

Fees: ($170) by check per each person 15 years of age or older and ($80) by check per each person under 15 years old. Payment should be made by certified check payable to IOM (International Organization of Migration) certified from the National Bank of Iraq. The National Bank of Iraq is located on Al-Sadoon St. – Ferdows Sq. (near the gas station / opposite to Al-Alawiah Club (Sheraton and Le Meridian Hotels).
Applicants must provide the following documentation when they appear for their examination:
· Recent passport sized photographs (4 copies)
· Color photocopies of passport (3 copies).
· Color photocopies of identification card ( 3 copies )
· Appointment letter from NVC showing that you have a scheduled visa interview (3 copies).
· Immunization records if available.
· All past medical reports and current medications, if any.

Fees: ($170) U.S. Dollars Cash per each person 15 years of age or older and ($85) U.S. Dollars Cash per each person less than 15 years old. U.S. Cash will be the only accepted form of payment..
When appearing for examination at the Wamar Clinic, applicants must provide the following documentation:
· Recent passport photographs according to attached instructions (4 copies )
· Color photocopies of passport (3 copies).
· Color photocopies of identification card ( 3 copies )
· Appointment Letter from NVC for scheduled visa interview at the Consular Section of the U.S. Embassy (3 copies).
· All past medical reports and current medications, if any.
وطبعا اني اتفاجئت وسألته كتله يابه لعد هم شلون دزولي بالمسج عنوان مركز الشفاء وارقام الاطباء حته اذا اريد اتصل بيهم وافحص يمهم؟؟؟؟؟ كال لان ال nvc يعتمدون على وثائق قديمه وهسه الشفاء صار غير مخول من قبل السفاره.
اني اريد رأيكم بهذا الحجي شتكولون صدك كذب؟؟ واريد نصيحتكم لان دخت وماعرف شأسوي؟؟ لان اني بصراحه ماريد اكون من الي يبثون اشاعات بس هو هذا الي صار !!!
